Subject: [PATCH] build: retain only needed explicit Qt includes for Shell 2.5

Shell 2.5 added the former compatibility includes to most files, changed
include ordering, and removed usagefmt.hpp. Retain the remaining direct Qt
type includes without trying to patch already-fixed or removed files.
